Please tell us if the disconnection of main electricity presents a clear risk to your (or that of someone in your household) health or wellbeing for the reasons of age, health, disability or severe financial insecurity.

We'll need your test location certificate for 3 or more gas bottles. Please email a copy of this certificate to firstname.lastname@example.org.
Please describe the Supplier sticker on the gas bottle.
We recommend you have at least two 45KG gas bottles to reduce the risk of running out. Simply let us know how many you would like. If you already have 45KG gas bottles or are wanting to source them from somewhere else, please select none.
This includes locked gates that need a key or security code and dogs—even if they are tied up.
